At Middlecoff Dental Group we make every effort to estimate the patient's co-payment prior to treatment.  However, because of the increasing number of insurance plans with which we must deal, it is not possible for us to be precise 100% of the time. Therefore, at the time of service, the patient pays his/her estimated co-pay. If insurance pays less than estimated the patient will receive a statement for any remaining balance or a refund if insurance pays more than estimated.

Middlecoff Dental Group participates with many insurance carriers, however all plans under the same carrier are not the same. We are not always able to find out every detail about your particular plan. We suggest that you contact your insurance carrier before you begin treatment so that wou will be aware of how your particular plan works. We will be happy to submit pre-treatment estimates for more accurate benefit calculations.

We will submit all insurance claims for you. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your policy has not paid within 30 days.

We accept all major credit cards, checks and cash for payment. We also accept CareCredit for interest free or extended payment plans. Credit approval is required and your application can be processed in office so treatment can be started the same day.
